The person making the intention would do so as follows: “I am entering into Ihram for Umrah and reciting the Talbiyah on behalf of Rasulullah ﷺ.” After making this intention, the Umrah will be performed without making any additional intentions for other acts to be performed on his behalf.

Abdullah ibn Abbas narrated:
A woman from the tribe of Khath’am said, “O Allah’s Messenger! My father is an old man and cannot perform Hajj and Umrah, nor can he sit firmly on the camel. Can I perform Hajj and Umrah on his behalf?” The Prophet ﷺ replied, “Yes, you may perform Hajj and Umrah on his behalf. Don’t you think that if your father had a debt, you would pay it off for him? Abdullah ibn Abbas narrated:
The Prophet Muhammad ﷺ said, “Whoever performs Hajj for Allah’s sake and does not have sexual relations with his wife, and does not do evil or sins then he will return (after Hajj free from all sins) as if he were born anew.” The companions asked, “O Allah’s Messenger! Should we perform Hajj every year?” He replied, “If you have the means to do so, then yes. And whoever performs Umrah on behalf of his parents, or performs Umrah on behalf of one of them, then he should fulfill their pledges and debts, and Allah will record a good deed for him for every step he takes.” (Bukhari)
